Documents you need
Bring original documents together with readable photocopies. Names, dates and record details should match across every document.
- Birth information form (सूचना फाराम) filled in and signed by the informant (father or mother, or a family member aged 18+)
- Copies of the parents' and the informant's citizenship certificates (DoNIDCR FAQ 13)
- Hospital birth report, or the child's vaccination card for a home birth (DoNIDCR FAQ 13)
- Police report, if the father is missing (DoNIDCR FAQ 13)
- Ward inquiry (सर्जमिन) form, if the ward needs more verification (DoNIDCR FAQ 13)
- Some wards also ask for the parents' marriage registration certificate (e.g. Panchkhapan Municipality, 2077 charter) or a current fiscal year's house/property-tax receipt (e.g. Aandhikhola Rural Municipality, 2073 charter)

Step-by-step process
- 1
Get the birth report from the hospital or health facility (if the child was born there); for a home birth, keep the vaccination card.
- 2
Within 35 days of the birth, go to the ward office of your permanent address, or to the local registrar's office where the birth took place (DoNIDCR FAQ 4).
- 3
Fill in the birth information form; the informant (father or mother, or an adult family member) signs it.
- 4
Within 35 days registration is free. After 35 days the event is still registered, but you pay the national late fee set in Schedule 3 of the National ID and Registration Rules 2077 (amount: confirm at the ward).
- 5
The ward registers the birth and issues the birth registration certificate, usually the same day.
- 6
Check the child's name, date of birth and parents' details before leaving - this certificate is used later for citizenship, the minor ID card and passports.

Fees and charges
Registration within 35 days
DoNIDCR FAQ 4: '३५ दिनभित्र निःशुल्क वा त्यसपछि बिलम्ब शूल्क तिरी दर्ता गराउन सकिन्छ'; Rules 2077 r.16(3): notice forms are received 'बिना दस्तुर' (without fee). Older local charters listed a fee anyway (e.g. Panchkhapan Municipality Rs 100, Rs 500 if born abroad, 2077 BS); if your ward asks for one, ask under which law.
Free
Late registration (after 35 days)
Set nationally, not by the local Finance Act: Act 2076 s.19(3) (registered 'तोकिए बमोजिमको विलम्ब शुल्क लिई') and Rules 2077 r.17(2) (late fee per Schedule 3, अनुसूची-३). The official online copy of the Rules ends before Schedule 3, so the amount could not be read. Older local figures (Aandhikhola Rural Municipality Rs 50, 2073 BS; local levels in Bara district Rs 700, reported 2022) are unverified and probably outdated.

Copy of birth certificate
Rules 2077 r.21(2): each copy costs the Schedule 3 fee; r.21(3): half the fee if the certificate was lost or not delivered for reasons beyond your control. Older local example: Panchkhapan Municipality Rs 300 (Rs 700 if born abroad), 2077 BS.

Important notes
- The 35-day free period and the late fee are national rules (Act 2076 and Rules 2077), so they should be the same at every ward; the late-fee amount itself was not readable online.
- Events from before the Birth, Death and Other Personal Events (Registration) Act 2033 took effect cannot be registered; the ward chair can certify the event instead (DoNIDCR FAQ 5).
- A birth to a woman in prison is registered by the local registrar of the mother's permanent address (Rules r.17(6)).
- A stillbirth, or a death soon after birth, can be reported with a health-institution document and the ward chair's recommendation (Rules r.25(4)).
- Only about 6% of children were registered within 35 days (2022 news); registering early avoids the late fee.
- DAOs later check that the birth is registered in the online civil-registration system when the child applies for citizenship (DAO Nuwakot).
