Documents you need

Bring original documents together with readable photocopies. Names, dates and record details should match across every document.

Application on the prescribed form with the ward recommendation (वडा सिफारिस)
Photocopy of the old certificate showing its number and issue date, or the damaged/old original itself
2 recent passport-size photos showing both ears (DAO Nuwakot; an older charter asks for 3)
If your record cannot be traced: a fresh Schedule-1 form, ward recommendation and citizenship copies of your parents or siblings (DAO Lalitpur)
If you have moved: migration certificate or land/building completion proof (DAO Nuwakot)
Married women: husband's citizenship and marriage registration; the husband must come in person (DAO Nuwakot)

Step-by-step process

Get a ward recommendation for a citizenship copy from your ward office.

Find a photocopy of your old certificate (number and issue date) or bring the damaged original.

Go to the DAO/AAO that holds your citizenship record with 2 photos and the documents.

Fix the revenue stamp on the application and submit it at the citizenship-copy (प्रतिलिपि) section.

The office checks your record (अभिलेख); if it cannot be found the file is handled like a fresh application using parents' or siblings' citizenship.

Collect the copy the same day once the file is complete.

If you no longer live in the issuing district, ask whether your current DAO can obtain your record; DAO Nuwakot lists migration/land proof for copies.
