02 · Prepare

Documents you need

Bring original documents together with readable photocopies. Names, dates and record details should match across every document.

  • Personal application clearly stating what is wrong and what it should be, with 2 photos (DAO Nuwakot)
  • Original ward recommendation (वडा सिफारिस)
  • Original citizenship certificate plus photocopy
  • Education certificate (SEE/SLC) showing the correct name/age — photocopy attached, original shown
  • School recommendation; DAO Lalitpur also lists education-office, local-body and police recommendations for name/surname/age corrections
  • To add a husband's name: marriage registration and husband's citizenship (older charter)
  • Divorced women restoring their parents' address: divorce registration, certified court decision and verification by a parent or sibling (older charter)

03 · At the office

Step-by-step process

  1. 1

Identify the document that shows the correct details (usually your SEE/SLC certificate). If the error is in the SEE certificate itself, correct it at the exam board first within the allowed period.

  2. 2

Get the ward recommendation and, for name/age changes, the school recommendation.

  3. 3

Write an application to the authorised officer (Chief District Officer) describing the error; attach photocopies and 2 photos.

  4. 4

Submit at the DAO holding your citizenship record with the original certificate and a Rs 10 revenue stamp.

  5. 5

The officer checks the evidence and may call witnesses or ask for a local/police report.

  6. 6

Collect the corrected certificate once approved.

06 · Read carefully

Important notes

  • SEE/SLC certificate correction window: DAO Lalitpur says only within 6 months of results; DAO Nuwakot's charter text mentions 5 years — confirm. After the window, DAO Lalitpur only issues a recommendation that both names refer to the same person.
  • Dalit applicants changing surname need a local-authority recommendation (DAO Lamjung).
  • Divorced or separated women may ask to change name, address or other details (DAO Lamjung).
  • After correcting your citizenship, correct your NID at the DAO's NID registration station (your NIN stays the same) and apply for a new passport, since a passport cannot be amended.
