02 · Prepare

Documents you need

Bring original documents together with readable photocopies. Names, dates and record details should match across every document.

  • Death information form filled in and signed by the informant
  • Deceased's citizenship certificate, or another identity document (DoNIDCR FAQ 25)
  • Informant's citizenship certificate (DoNIDCR FAQ 25)
  • Hospital death report, if the death happened in a health facility (DoNIDCR FAQ 25)
  • For a foreign national: passport and a recommendation from the diplomatic mission (DoNIDCR FAQ 25)
  • Proof of relationship to the deceased, e.g. a relationship certificate (asked by some wards; not in the DoNIDCR list)
  • If the deceased had no citizenship (Aandhikhola: also if unmarried): local inquiry letter (सर्जमिन)
  • If the deceased received a social-security allowance: details needed to stop the allowance (Panchkhapan)
  • Current fiscal year's property-tax receipt (some wards)

Step-by-step process

  1. 1

Within 35 days of the death, the informant (a parent, spouse, son or daughter, or an adult family member) goes to the ward office of the permanent address, or to the local registrar's office where the death occurred (DoNIDCR FAQ 4).

  2. 2

Fill in the death information form and attach the documents.

  3. 3

If the deceased had no citizenship, the ward may carry out a local inquiry (सर्जमिन).

  4. 4

Within 35 days registration is free. After 35 days the event is still registered, but you pay the national late fee set in Schedule 3 of the National ID and Registration Rules 2077 (amount: confirm at the ward).

  5. 5

Collect the death registration certificate, usually the same day.

Fees and charges

Registration within 35 days

DoNIDCR FAQ 4: '३५ दिनभित्र निःशुल्क वा त्यसपछि बिलम्ब शूल्क तिरी दर्ता गराउन सकिन्छ'; Rules 2077 r.16(3): notice forms are received 'बिना दस्तुर' (without fee). Older local charters listed a fee (Aandhikhola Rural Municipality Rs 50, 2073 BS; Panchkhapan Municipality Rs 100, Rs 700 if the death occurred abroad, 2077 BS); if your ward charges one, ask under which law.

Free

Late registration (after 35 days)

Set nationally, not by the local Finance Act: Act 2076 s.19(3) (registered 'तोकिए बमोजिमको विलम्ब शुल्क लिई') and Rules 2077 r.17(2) (late fee per Schedule 3, अनुसूची-३). The official online copy of the Rules ends before Schedule 3, so the amount could not be read.

Not published - confirm at the office

Copy of death certificate

Rules 2077 r.21(2): each copy costs the Schedule 3 fee; r.21(3): half the fee if the certificate was lost or not delivered for reasons beyond your control.

06 · Read carefully

Important notes

  • The 35-day free period and the late fee are national rules (Act 2076 and Rules 2077), so they should be the same at every ward; the late-fee amount itself was not readable online.
  • Events from before the Birth, Death and Other Personal Events (Registration) Act 2033 took effect cannot be registered; the ward chair can certify the event instead (DoNIDCR FAQ 5).
  • A death soon after birth, or a stillbirth, can be reported with a health-institution document and the ward chair's recommendation (Rules r.25(4)).
  • A parent's death registration is later required when a child applies for citizenship (DAO Lalitpur) or through the mother's name if she has died (DAO Ilam).
  • Register promptly so that any social-security allowance is stopped correctly (Panchkhapan lists allowance details as a requirement; the allowance procedure requires a death to be reported within 35 days).
