Documents you need

Bring original documents together with readable photocopies. Names, dates and record details should match across every document.

  • Application in the procedure's Annex 1 format, with two copies of supporting documents
  • Recent passport-size photo(s), showing the disability where possible (number not specified in the procedure)
  • Proof of disability, for example a certified copy of a disability record or medical document
  • One supporting document: citizenship certificate, land ownership certificate, education certificate, voter ID, appointment letter, or a recommendation from a disability organisation
  • Guardian's name (recorded on the form) where a guardian applies

03 · At the office

Step-by-step process

  1. 1

Go to the ward office of your permanent address and fill in the application (Annex 1 format).

  2. 2

The ward tells you within 3 days if any document is missing.

  3. 3

The ward forwards the application with its recommendation to the municipality or rural municipality within 3 days. It may consult a doctor, a specialist or a disability-organisation representative.

  4. 4

Visible disabilities are assessed on the spot; other cases go to the local coordination committee (स्थानीय समन्वय समिति), which includes a doctor, a school head or resource person, and persons with disabilities.

  5. 5

    The committee decides the class: क/A red (पूर्ण अशक्त, profound), ख/B blue (अति अशक्त, severe), ग/C yellow (मध्यम, moderate) or घ/D white (सामान्य, mild).

  6. 6

The chief administrative officer (or a delegated officer) signs and issues the card, free of charge.

  7. 7

If you disagree with the decision, appeal to the municipality or rural municipality within 15 days of being notified.

  8. 8

If the card is lost or damaged, apply at the ward for a free duplicate.

Important notes

  • Only red (full) and blue (severe) card holders are eligible for the disability social security allowance: Rs 3,990 a month for red and Rs 2,128 for blue (ministry's official rate page; news reports giving Rs 2,218 for blue are wrong).
  • The card is bilingual, with Nepali on one side and English on the other, and looks like a citizenship certificate.
  • Holders of cards issued before the 2075 procedure had to replace them within one year.
  • Mobile disability-card camps must be held at least once a year in remote areas.
  • The Roshi RM text is an OCR copy and local procedures can differ slightly; confirm with your municipality.
  • The ministry posted a procedure for disability-targeted programmes at the local level, 2083 (8 Asoj 2083); not reviewed.
